Improving life of patients with chronic illness community forum

FREEPORT, Maine –  Southern Maine Area Agency on Aging, MMC PHO and MaineHealth will hold a community forum on “Partnering to Improve Care: Improving the Quality of Life for Patients Living with Chronic Illness” 1-3 p.m. Monday, Dec. 9, at Freeport Community Center, 53 Depot St. This program will present successful approaches to navigating the healthcare system and provide information about the innovative partnerships and services being offered in our community. Speakers will include: Larry Gross, executive director, Southern Maine Area Agency on Aging; Peggy Haynes,  director, Elder Care Services, Partnership for Healthy Aging, MaineHealth; and Chris Murry Jr., manager of Special Initiatives, MMC Physician-Hospital Organization
