Bangor, ME – Recent advancements in healthcare delivery models have shown the benefits of coordinated care focused on the individual needs of each patient. This care starts with the relationship between a patient and their primary care team. Eastern Maine Medical Center’s new family medicine practice expands access to these primary care services in Hampden. The practice is located in the plaza at 7 Main Road North in Hampden.
The new practice builds on the experience gained by EMMC’s participation in the Beacon Grant and the Pioneer Accountable Care Organization through EMHS. Patients have access to care coordinators who have expertise in chronic care management and experience building customized treatment plans based on the needs of each patient.
The Hampden team will begin caring for patients from other EMMC family medicine offices who wish to transfer their primary care to Hampden in August. The practice will open to new patients on September 16. Patients will be seen Monday through Friday from 7:30 am to 5:15 pm.
