Should all baseball players be tested monthly for performance-enhancing substances?

Former Red Sox star and current Dodger slugger Manny Ramirez tested positive for a banned substance and will be suspended from playing for 50 games. How can this stain on the sport – which extends far beyond Mr. Ramirez – be removed? Should players be tested every month? Should a positive test mean a permanent ban from the game? Should criminal charges be created?
